5. Arrest/Conviction Report and Certification form 

6. PA Sexual Misconduct/Abuse Disclosure Release (Electronic Form sent to you after offer of employment)
This electronic form is required for employees and independent contractors.  You are required to complete an electronic form for your current employer, any schools in which you work, or all employers where you worked with children.  The district will send electronic forms to your current and former employers.  

7. 49 CFR Part 40 Drug and Alcohol Testing(For Bus & Van Driver)

* As of December 2014, Pennsylvania Act 153 of 2014 requires that school district employees obtain new clearances every 60 months.  Once hired by the district, you will be required to obtain new clearances every 60 months from the date of the oldest clearance.
